About Us
Weld County Prevention Partners exists to engage individuals, organizations and schools in strategies for preventing substance abuse and families and youth to consider healthy choices and solutions. Through a comprehensive community assessment and strategic planning process WC has a Prevention Plan that includes proven strategies for implementation to improve the lives of its residents.
Weld County Prevention Partners works closely with 1451 Collaborative Management Program, which serves as the Prevention Policy Board of the coalition, to leverage resources and impact the goals of the Prevention Plan. Weld County Commissioners and key agency partners throughout Weld County cam together and formed the Weld County HB04-1451 Collaborative Management Program on July 1st 2005. Since then the group has grown and become more invovled in the coordination of programs and services throughout Weld County with an emphasis on:
- Reducing duplication and eliminating fragmentation of services provided;
- Increase quality, appropriateness and effectiveness of services provided;
- Evaluate progress;
- Encouraging cost-sharing among service providers; and
- Ultimately leading to better outcomes and improved financial effciencies for services provided to children and families that would benefit from mult-agency care.
